Customizing your template with blocks

After creating your template, you can use blocks in the template builder to structure and customize it for your practice's needs. This guide explains the types of blocks available and how to add, edit, organize, and manage them. We'll cover:

Important: Any changes made in the template builder must be saved. Be sure to click Save in the top right corner as you work to avoid losing changes.


Understanding different block types

Templates are built using 2 types of blocks: content blocks and format blocks

  • Content blocks collect responses
  • Format blocks help organize and structure the template

Content blocks

Content block Description
Short response
  • Provides a prompt and a single-line text field for brief responses
Long response
  • Provides a prompt and a larger text field, recommended for open-ended responses
Single option
  • Provides a prompt with multiple choices; only one option can be selected (radio buttons)
Multiple options
  • Provides a prompt with multiple choices; more than one option can be selected (checkboxes)
Dropdown
  • Provides a prompt with multiple choices in a dropdown menu; only one option can be selected
Date
  • Provides a prompt and a date picker (MM/DD/YYYY format)
eSignature
  • Adds a prompt that collects an electronic signature, including date, time, and relationship to the client (available for intake forms only)

Format blocks

Format block Description
Group
  • Creates a labeled section that visually groups related content blocks
Heading
  • Adds a section heading without grouping blocks
Description
  • Displays instructional or contextual text without requiring a response
Divider
  • Adds a horizontal line to separate sections

Adding blocks

To begin customizing your template, add content blocks or format blocks from the left column of the template builder.

When you add a block, it appears at the bottom of the template. You can also select a block from the left column and drag it to a specific position.

When a block is selected for editing, the remaining blocks collapse so you can focus on one block at a time.

Adding content blocks

All content blocks include an [Insert prompt and question] field where you can enter the prompt that will be displayed.

  • Short response blocks: You can add suggested responses by selecting Add suggested response. These suggestions appear in a dropdown when the field is selected.

  • Single option, Multiple options, and Dropdown blocks:

    • Enter each response option in the Insert option field.
    • Select Add option to include additional responses.

For Single option and Multiple options blocks, you can also attach a text field to a response:

  1. Select Text response under the response option
  2. Enter hint text to guide the response

Note: You can preview how your template appears at any time by selecting Preview in the top right corner.


Editing and moving blocks

To edit a block:

  1. Select the collapsed block to expand it

  1. Make the changes as needed
  2. If you want to change the block type, use the dropdown menu at the top of the block

To move a block:

  1. Click and hold the 6 dots at the top of the block
  2. Drag the block to a new position

Using Group blocks

You can use a Group block to organize related prompts into a single section.

After adding a Group block from the left column:

  • Drag content blocks into the dark gray area beneath the Group block

  • Blocks within a group are outlined in dark gray

To remove a block from a group, drag it outside the grouped area.

Important: If you delete a Group block or change its block type, the blocks inside the group are also deleted and will need to be recreated.

The Heading block can also indicate section, but blocks under a Heading aren't grouped.


Requiring, duplicating, and deleting blocks

To require a response, enable the Required toggle on the content block.

Required prompts will display a red asterisk.

Note: Questions without responses won’t appear on completed forms or documents.

To duplicate a block, select the duplicate icon.

Keep in mind that deleted blocks can't be restored. If needed, you'll need to recreate the block. To delete a block, select the trash icon.

Previewing your customized template

To preview how your template will appear when in use, click Preview in the top right corner.

Be sure to click Save before previewing so your latest changes are included.

While previewing, you can click the printer icon to print the template or download a PDF copy.

Important: Any changes made in the template builder must be saved. Be sure to click Save in the top right corner as you work and when you're finished to avoid losing changes.
