It evaluates notes you've written against a rubric of industry standards for clinical documentation and provides structured, actionable feedback to help you build documentation skills over time.

Getting started with Note Checker
Note Checker evaluates progress notes you've written against a rubric that covers industry standards for clinical documentation. It provides feedback to surface areas where your note could be strengthened before you submit the note to your supervisor for review.
Note: Unlike Note Taker, Note Checker won’t generate content for a progress note, or edit your progress note draft directly. For more information, see What's the difference between Note Taker and Note Checker?
Feedback is organized into the following areas:
- Client's perspective: Accurate documentation of client's self-reported symptoms, concerns, mood, and subjective experience using direct quotes when possible
- Observable information: Professional observations of client's appearance, behavior, mood, affect, speech, thought processes, and cognitive functioning
- Clinical interpretation: Professional analysis integrating subjective and objective data, progress toward goals, clinical impressions, and risk assessment
- Treatment interventions & Next steps: Specific interventions provided, homework assignments, medication considerations, next appointment, and modifications to treatment plan
- Medical necessity and functional impairment: Clear documentation of why services are medically necessary and how symptoms impact client's daily functioning, work, relationships, or safety
- Treatment goal progress: Specific documentation of progress toward established treatment goals with measurable indicators and timeframes
- Professional standards and compliance: Adherence to professional writing standards, confidentiality requirements, timeliness, and regulatory compliance
These categories represent what's generally expected in well-documented clinical notes. Note Checker uses them to identify where your note may benefit from additional detail, clarity, or structure.
Enabling Note Checker
Once Care Aide is enabled for a supervisee, the Account Owner can select whether that supervisee uses Note Checker or Note Taker as part of Care Aide.
To first enable Care Aide, see Managing Care Aide in your account.
Once Care Aide is enabled for the supervisee, select Note Checker under Supervisee feature access, then click Save.
Selecting Note Checker means the supervisee won't have access to Note Taker or features that rely on Note Taker within the Care Aide add-on. For more information, see What other Care Aide features can I use with Note Checker?
Checking your progress note with Note Checker
After you’ve written and saved your progress note, you can click Note Checker to review your progress note:
A right panel will open and begin checking your note.
Note: Note Checker won’t run if a progress note hasn’t been written and saved first.
Once the review is completed, feedback will appear in the Note Checker panel.
You can rewrite your progress note based on the feedback provided, and save it to retain the updates. AI can make mistakes, so always use your own clinical judgment to decide which of Note Checker’s feedback to include. For more information, see What if the feedback doesn't seem accurate?
Rerunning Note Checker
After rewriting or editing your progress note, you can use Note Checker again to access updated feedback.
Note: The rewritten progress note must be saved before rerunning Note Checker. If the note is shared with the supervisor or signed and locked, Note Checker won’t be able to run for the note.
To do this:
- Select Note Checker
- Click Check again
You can use Note Checker up to 5 times on a single progress note. Each time you click Check again, the counter decreases by one. Your remaining uses will be displayed within the Check again button.
Once you've used all 5 attempts, you won't be able to recheck the progress note.
If you make edits without clicking Check again, the Note Checker panel will indicate that the feedback is no longer up to date due to changes in the progress note.
Note: You’ll always receive suggestions from Note Checker, as it’s designed to help you find areas for growth in every note. You can choose to stop rerunning Note Checker at any time when you feel it is ready for your supervisor to review.

What's the difference between Note Taker and Note Checker?
Note Taker generates a full progress note draft based on session content.
Note Checker doesn’t generate a draft or automatically edit anything you’ve written. It reviews your progress notes and provides feedback based on industry standards for clinical documentation. It's designed to help you build documentation and clinical skills by evaluating what you’ve written.
Can I use both Note Taker and Note Checker?
No. Note Checker and Note Taker are mutually exclusive for supervisees. The Account Owner can determine whether Note Taker or Note Checker will be enabled for supervisees. For more information, see Enabling Note Checker.
Note: If the Account Owner switches from one tool to another, the update will apply to new notes you start.
What other Care Aide features can I use with Note Checker?
Note Checker is designed to help you develop strong documentation habits by evaluating notes that you’ve written yourself. Because of this focus on building your skills, the features available alongside Note Checker are limited to tools that support preparation and skill development rather than full content generation.
With Note Checker enabled, you can access pre-session summaries to help prepare for your upcoming session.
Other Care Aide features won't be available, such as Intake Summaries, Session Sidekick, Client Summaries, and Treatment Planner. These tools rely on Note Taker being enabled to generate content. Because Note Checker and Note Taker are mutually exclusive, these tools aren't available while Note Checker is enabled.
The Account Owner can switch a supervisee’s access to Note Taker in their practice settings at any point. Once Note Taker is enabled within Care Aide, the full set of Care Aide features becomes available.

What note types does Note Checker work with?
Note Checker works with all progress note templates across specialties. It also works with progress note templates that were created from scratch, or customized from a pre-built template.
Note Checker doesn't work for other documentation types, such as diagnosis and treatment plans or intake forms.
What if the feedback doesn't seem accurate?
Note Checker is assistive and may produce feedback that's incomplete or missing context. It evaluates your note against a general rubric, which means some suggestions may not apply to your specific session or client. Use your clinical judgment when deciding which suggestions to incorporate. If you'd like to share feedback with our team about the feature, see Leaving feedback on features.
Can my supervisor see my Note Checker results?
No. Supervisors can't see whether Note Checker was used on a specific note or what suggestions were provided. Note Checker only provides feedback, and won’t directly alter any of your written content.
