The Progress note compliance Insights page helps you track how long it takes clinicians to sign and lock progress notes. To access this Insights page, navigate to Analytics > Insights > Progress note compliance.
The Progress note compliance page includes two metrics: On-time note completion rate and Time to completion.
Important: Clinician-level insights are intended to support practice management, administrative efficiency, and clinician wellness. Please ensure your use of this data complies with applicable employment laws and your internal practice policies.
In this guide, we’ll cover:
- Overview of the On-time note completion rate metric
- Setting a definition and goal for the On-time note completion rate metric
- Overview of the Time to completion metric
- Highlights for the Progress note compliance page
- Scenarios that may affect the Progress note compliance page

Overview of the On-time note completion rate metric
The On-time note completion rate metric appears by default when you view the Progress note compliance page. This metric shows the percentage of progress notes that were signed on-time for all sessions scheduled during the selected timeframe.
Note: Sessions without signed progress notes aren’t included in this metric.
To define what your practice considers on-time, see Setting a definition and goal for the On-time note completion rate metric.
Use the From dropdown to change the metric's timeframe.
If your practice has multiple clinicians, use Filter by to include data for selected clinicians. You can also use View by to group the data by time or clinician.
The graph displays different information depending on whether you group the data by time or clinician. The sections below explain how to interpret each view.
Viewing by time
By default, the metric groups data by time. In this view, the on-time note completion rate is shown for each period within the selected timeframe (day, week, or month).
You can hover over a point in the graph to view details for that period.
The table below explains the data shown when you hover:
| Data point | Definition |
|---|---|
| On-time note completion rate |
The percentage of progress notes that were signed on-time out of all signed progress notes for sessions scheduled for the time period (day, week, or month). For example, a progress note may be considered on-time if it’s signed and locked within 2 days after a session’s end. Progress notes that were signed within that timeframe are included in the on-time note completion rate. To learn how to define on-time for progress notes, see Setting a definition and goal for the Progress note compliance page. |
| On-time notes (≤ days) | The number of progress notes that were signed on-time during the time period. |
| Total notes completed | The total number of progress notes signed for sessions scheduled for the time period. |
The table under Data also shows Late notes, which are notes included in Total notes completed that weren’t signed on-time.
Viewing by clinician
If your practice has multiple clinicians, you can view the metric by clinician by selecting Clinician from the View by dropdown. This will display each clinician’s on-time note completion rate within the selected timeframe.
Use the Sort by dropdown to sort the data by Highest rates, Lowest rates, or Clinician name.
You can hover over a bar in the graph to view details for a clinician.
The table below explains the data shown when you hover:
| Data point | Definition |
|---|---|
| On-time note completion rate | The percentage of the clinician’s progress notes that were signed on-time out of all their signed progress notes for sessions scheduled for the selected timeframe. |
| On-time notes (≤ days) | The number of the clinician’s progress notes that were signed on-time for sessions scheduled for the selected timeframe. |
| Late notes | The number of the clinician’s progress notes that weren’t signed on-time for sessions scheduled for the selected timeframe. |
| Total notes completed |
The total amount of the clinician’s progress notes signed and locked for sessions within the selected timeframe, regardless of whether the notes were on-time. Note: This is also the sum of On-time notes and Late notes. |
Setting a definition and goal for the On-time note completion rate metric
You can define how soon after a session a progress note must be signed to count on-time. You can also set a goal for how many progress notes are signed on-time. This goal will only impact the On-time note completion rate metric.
To set a definition and goal:
- When viewing the On-time note completion rate graph, click the gear icon in the top right
- Under Definition, enter the number of days clinicians have to sign a progress note after a session ends
- You can select a number up to 7
- To set a goal, toggle Goal on and enter the Min acceptable rate of progress notes that should be completed on-time
- Click Save
Your goal combines the on-time definition and the minimum acceptable rate of progress notes signed on-time.
For example, a progress note may be considered on-time if it’s signed within 3 days after a session, and you may have a minimum acceptable rate of 50%. Your goal would be for 50% of progress notes to have been signed 3 days after a session was held.
When you set a goal, a goal line appears on the graph. You can use this line to compare the actual on-time note completion rate with your goal.
Overview of the Time to completion metric
The Time to completion metric indicates how many notes were signed during a window of days after a session ends (i.e. 7+ days after a session’s end).
To view this metric, click the dropdown under Show me and select the Time to completion metric.
Note: You can use From to change the timeframe and Filter by to include data from selected clinicians.
You can hover over a bar in the graph to view details for that completion window.
The table below explains the data in the tooltip:
| Data point | Definition |
| % of notes completed in window | The percentage of notes that were signed within that window out of all progress notes signed for sessions scheduled for a date during the selected timeframe. |
| Notes completed in window | The number of progress notes that were signed within that window (e.g. 2-3 days). |
Highlights for the Progress note compliance page
You can see the following highlights for the Progress note compliance page:
- Rate of change: Indicates whether your practice’s on-time note completion rate increased or decreased compared with the previous week, month, quarter, or year
-
Goal: Indicates whether the current on-time note completion rate is behind your practice’s minimum acceptable rate of notes completed on-time
- You can also see whether certain clinicians’ rates are behind the practice’s goal

Scenarios that may affect the Progress note compliance page
Below are a few scenarios that may impact the Progress note compliance page.
Note: For additional scenarios that may affect a metric, click the question mark icon or click Metric guide in the top right corner of the page.
What if a progress note is unlocked and re-signed?
To calculate completion time, Progress note compliance metrics measure the time between the end of an appointment and the timestamp recorded when a clinician selects Sign and lock for the progress note.
If the progress note is unlocked and signed again, a new timestamp will be generated. The final timestamp will be counted toward progress note metrics.
What if I save a progress note immediately, but sign it on a later date?
If there’s a long duration between when a progress note is saved and when it’s signed, this may skew progress notes to a certain window. For example, see the scenario below:
- A clinician has an appointment on May 4
- They save their progress note for the appointment on May 5 without signing it
- They sign the progress note at the end of the month
The progress note in this scenario would be included in the 7+ days window of the Time to completion metric.
Note: Sessions without signed progress notes aren’t included in the Progress note compliance metrics.
How are notes completed by pre-licensed clinicians included in metrics?
If the clinician that signs a progress note is pre-licensed and has a supervisor, only the pre-licensed clinician’s signature will count toward progress note metrics.
Note: To learn more about signing notes as a supervisor, see Reviewing and signing documentation under supervision.
How are notes for group appointments included in metrics?
For group appointments, only the progress note for the entire appointment will count toward Progress note compliance metrics.
