Saving a Discharge Summary for a client

When you discharge a client from your practice, you can record their status in their profile by creating a Discharge Summary. SimplePractice includes a Discharge Summary template, which you can customize and use for your clients.


Adding the Discharge Summary template

By default, the Discharge Summary Note is available as a template in your account. To confirm this, navigate to Settings > Documentation > Template library and view the Other documents section.

Note: To preview this template, click the eye symbol next to Discharge Summary Note.

If the template isn’t already listed, you can add it from the pre-built templates:

  1. Navigate to Settings > Documentation > Template library
  2. Click Browse pre-built templates
  3. Scroll to the Other documents section
  4. Select + Add next to Discharge Summary Note

Note: To preview the Discharge Summary Note before adding it, click the eye symbol next to + Add.

You can customize the template based on your practice’s needs. For more information, see Creating a new template and accessing the template builder.


Creating a Discharge Summary in a client's profile

To create a Discharge Summary for your client:

  1. Navigate to the client's Overview page
  2. Click New > Other document

  1. Select Discharge Summary Note from the dropdown menu

  1. Enter the necessary information
  2. Click Save

After completing the Discharge Summary Note, click Sign in the top right corner. Review your signature and click Sign and lock.
