You can add and keep track of progress notes, psychotherapy notes, and diagnosis and treatment plans in the SimplePractice for Clinicians mobile app. Below, we'll cover:

Adding a progress or psychotherapy note on iPhone/iPad
After adding an appointment, you’ll have the option to add a progress note or psychotherapy note.
Note: You can also view progress notes and psychotherapy notes for prior appointments by viewing the appointments from the calendar.
To add appointments and view them from the calendar, see Managing appointments on iPhone/iPad.
To add a progress note or psychotherapy note to an appointment on the app:
- Tap the appointment in the Calendar
- Select Add + next to Progress note or Psychotherapy note
- If you’re adding a progress note, tap the two arrows next to Template and select your template
- You can choose from all of the active progress note templates in your SimplePractice account
- Fill out your progress note or psychotherapy note, as needed
- If available, you can also select Load previous note to populate and edit the content of the client’s last progress note
- Tap Save
After saving a progress note, you have the following options:
- Choose the pencil icon to edit the note
- Choose the square and arrow icon to copy, share, or print the note
- Select Sign to sign and lock the note
Note: To use Note Taker to automatically create progress notes within the mobile app, see In-person Note Taker on iOS.
You can also add and view progress notes through the Documentation option in the client’s Overview page.
After tapping Documentation, you’ll be able to view the progress note and psychotherapy note status for all of the client’s individual appointments. If a note has been created, you’ll be able to tap View for that note.
Note: A padlock symbol next to View indicates whether a note has been signed and locked.
If a note hasn’t been added to an appointment, tap Add next to the note you’d like to create.
Note: You won’t be able to add notes for group appointments through the mobile app. You can use the browser version of SimplePractice to add these. For more information, see Adding progress notes for group appointments.
Viewing diagnosis and treatment plans on iPhone/iPad
You can view a client’s latest diagnosis and treatment plan by navigating to the client’s Overview page, and tapping Diagnosis and treatment plan.
You can also view the client’s other other diagnosis and treatment plans:
- Tap Documentation on the client’s Overview page
- Select Diagnosis and treatment plan to view the plan
Note: There is no way to add or edit a diagnosis and treatment plan on the mobile app. To do this, use the browser version of SimplePractice.
Managing chart notes on iPhone/iPad
You can add a chart note for the client. To do this:
- Navigate to the client’s Overview page
- Tap + Create chart note
- Add the contents of the note
- If you’d like to adjust the date and time of the chart note, you can tap the date and/or time fields
- Tap Save
To view, edit, or delete an existing chart note:
- Tap Documentation on the client’s Overview page
- Select Chart note to view the chart note for a certain date
- Tap Delete note to delete the note, or select Edit to edit the note
Filtering for documentation on iPhone/iPad
If you'd like to view certain documentation on the mobile app for a client, you can tap the filter icon at the top right of the Documentation page.
You can select the following types of documentation to filter:
- All items
- This is selected by default
- Appointments
- This will filter the Documentation page for available progress notes and psychotherapy notes for each appointment
- Diagnosis and treatment plans
- Chart notes
You can also select Sort to filter for documentation based on a certain timeframe.
The default timeframe is All time.
Once you've selected your preferred filters, tap Apply filters. If you'd like to reset to the default filters, tap Reset.
